How do I schedule an appointment?

To schedule an appointment, please call the office at 7028430551 or use our book now link on our website under the services link. You can also walk-in during office ours and we will be happy to assist you. 

What Should I expect during my first visit?

During your first visit, you will be required to complete your registration forms, consent forms, intake questionnaire. Please arrive 30 mins prior to your scheduled appointment to complete your paperwork. This will enable you to be seen at your scheduled time without much delay. Once your paperwork is completed, insurance verified and co-pay collected if required, you will then be checked in to be seen by one of our caring and supportive providers. 

What happens during the consultation phase?

Once you are called by one of our caring and supportive provided, working together with you, your goals will be identified, a unique, holisitc, treatment plan will be formulated with your consent and appropriate referral will be provided to meet any other health need you may be concerned about. 

What should I expect at the end of my appointment?

Once you and your provider have completed your initial appointment, a follow-up appointment will be schedule to continue monitoring your response to treatment plan formulated. During your follow-up appointment, you discuss your concerns with your treatment plan, response and new concerns. 

I can't make my appointment, what should I do?

We strongly encourage at least a 24 hours notice for appointment cancellations or to reschedule to avoid a NO CALL NO SHOW FEE of $25 for follow up appointments and $50 for new patient appointment. To  cancel or to reschedule an appointment, please call the office at 7028430551.

It is after business hours and I am out of my medications, what do I do?

We strongly encourage you to keep your appointment as scheduled as medications are only refilled until your next scheduled appointment. We do understand that emergencies do happen, but if you miss your appointment, please call the office as we can provide you with a 7 days refill until your next scheduled appointment. However, if it is after business hour, please go to the nearest urgent care for your medication refill and call the office during the next business hour for assistance. 

It's after business hour and I feel suicidal, what should I do?

We are sorry we are not available to assist you during this moment. Please call the National suicide prevention Lifeline at  18002738255  or go to your nearest Emergency room.
